Wednesday: 1pm till 9pm Appointments: Patients are seen by appointment only. To set up an appointment call 856-6166. If you are unable to keep a scheduled appointment kindly give us 24 hour notice. This courtesy makes it possible for us to schedule another patient who desires your appointment time. We make every effort to be on time for our patients and ask the same courtesy to us. In the event that we are delayed due to unexpected circumstances or emergency, we will attempt to advise you as soon as possible. Please remember that when dealing with people and dental procedures things don't always go as planned. We do our best to allow time for such things but in spite of our best efforts we will occasionally run behind schedule. Emergency Care: If you develop a problem requiring emergency treatment, please call us immediately so that we may arrange to see you. If the emergency arises at a time other than during regular office hours, please call the office and the voice message will give you instructions on how to reach us. Maintenance: We firmly believe in the importance of regular preventive dental health care. Although the responsibility of returning for this treatment lies with you, we will call or send a postcard when it is time for your preventive care appointment. When you receive this notice , please call or stop in to make an appointment. Financial Policy: As with any other business, payment is expected at the time service is rendered. In the event that you require extensive treatment we can arrange a payment plan for you. This payment plan will require an initial down payment, the balance being divided into interest free monthly installments. Any dental service involving a lab fee will require a 25% down payment. Insurance: Excluding HMO, PPO or DMO plans we accept all major dental insurance carriers. As a courtesy to our patients we will complete and file insurance forms relative to dental services. Our office is fully computerized and we usually do not need your insurance carriers forms. Please remember that most dental plans do not cover services at 100%. If your carrier is in our system we can tell you what they cover at your first visit. If we do not have that carrier on record it wont take us long to determine your benefits. During the course of treatment you will receive monthly statements for the portion of fee not covered by you insurance carrier. Patients are expected to begin payment on their portion of the balance at the time treatment begins.
